Ages 25-30: Permanent Lip Augmentations and Injectables

Generation Yers are significantly more open and less conservative than Generation X-ers. This variance in personality shows when it comes to the cosmetic procedures they decide to get done. While they generally are more open to body-enhancing procedures, like breast augmentations and liposuction, they tend to follow the same trend when it comes to the treatments that combat the signs of early aging.  Having less of an issue with going under the knife, millennials get permanent lip augmentations in lieu of temporary fillers. Ages 40-50: Eye Surgery and Mid Facelift

Time is your skin’s worst enemy when you hit the 40+ year mark. Everything about our skin changes. The high levels of collagen that give our youthful skin its elasticity begins to breakdown causing skin to droop and wrinkle. The layer of fat that sits just beneath the skin and gives the face a lifted voluminous look also begins to thin out and deplete. This adds to the thinness an aged face usually has. As a result of these effects, the skin surrounding the eyebrow and eyelid begin to droop causing a hooded eye effect. The bottom of the eye isn’t any better; ligaments in the area begin to relax, causing fat deposits to shift. This makes the eyes look droopy and constantly tired. During the 40-50 age range, many Generation X-ers come in to correct this. They usually opt for eye surgeries such as a brow lift, upper eyelid surgery, lower eyelid surgery and forehead lifts. For more severe aging cases, Gen X patients ask for a mid-facelift which is basically a combination of all the above procedures to correct the eye area. Transumbilical Breast Enhancement in NYC Tri-state Area

Breast Augmentations are the nation’s most popular cosmetic surgical procedure. Women are often born with a need and desire to feel sexy and good about themselves- unfortunately not all women are born with natural features that make them feel that way. More and more women are turning to surgery to obtain their personal desired aesthetic. As the number of surgeries continues to grow, so does the advancement in technique and the technology behind breast implants. There is now much more thought that goes into your surgery besides choosing the size in which you want your breasts to be or the substance in which they are going to be filled with.

Premier plastic surgeon, Dr. Robert Vitolo pioneered the development of the revolutionary scar-less breast augmentation. The Transumbilical Breast Enlargement better known as “TUBA” is a safe and effective method to performing a breast enhancement that produces attractive results in a shorter time period for the procedure, also providing a shorter recovery period as well. This revolutionary technique is done within 30 minutes and patients are able to return back to work within 2 days of their procedure. Rather than the traditional breast enlargement procedures that would be done through the areola leaving patients with bigger breasts and big scars, the Transumbilical Breast Enlargement is done by making incisions within the depths of your belly button leaving you with virtually invisible scars.

Dr.Vitolo uses a device called an introducer that is inserted through the belly button causing minimal trauma to the tissue. This introducer tunnels into the subcutaneous tissue beneath the breast tissue. A thin pencil shaped camera called an endoscope is inserted into the introducer so that Dr. Vitolo can accurately place the implant in the desired position. Once the introducer is removed a filler tube is attached to a self sealing valve in the implant.  All air from the implant is removed so that it becomes flat. The flattened implant is then rolled up tightly and slipped up the tunnel that has been made by the introducer until it lies in the pocket underneath the breast. The filler tube extends out through the incision in the navel. Using a special syringe device, the implant is filled with sterile saline solution. As the implant is filled, it unrolls and creates a pocket for itself that is the exact size desired. The incision of the belly button is then closed with a dissolving suture, and the patient is placed in mildly compressive dressing for 24 hours.

Types of Rhinoplasty in NYC

Reduction Rhinoplasty

A significant proportion of patients seeking rhinoplasty surgery are concerned that the sheer size of their nose is disproportionate with their facial features. Areas of particular concern are the presence of a bump on the bridge of the nose, excessive width of the bridge tip of the nose, and possibly excessive width of the nostrils.

Part of the consultation process is to help determine which features contribute more significantly to this impression, and therefore which should be addressed as a priority.

Using computer simulation in the process of assessment, makes it easier to appreciate the degree of change required in order to achieve balance, but also the characteristics of the nose shape that fits nicer with the rest of the features.

The bump on the bridge of the nose is primarily made of cartilage (lower two thirds) and only the top third is due to the nasal bones. Reducing the bump can target either a straight profile, a slightly concave one or in some instances a slightly convex one, but not as prominent.

Reduction in the tip of the nose requires a more complex combination of lowering the cartilages (that decrease projection) and reshaping that allows for a narrower, more and delegate or more defined to contour.

In order to achieve more significant correction and control for the shape of the tip of the nose, it is often necessary to use an open tip approach during the rhinoplasty, and combine it with some thinning of the skin of the tip and supra-tip (area just above the tip).

When the nostrils are relatively wide, an alar base reduction is the procedure capable to achieve and narrowing effect, and involves removing a wedge of tissue in the area where the nostrils meets the cheek. Inevitably this leads to a fine scar to the area.

One of the main conflicts in achieving satisfactory results with reduction rhinoplasty is related to the combination of reduction in the tip projection and narrowing of the tip the context of relatively thick nasal tip skin.

Augmentation Rhinoplasty

Augmentation rhinoplasty refers to the type of surgery where the dimensions of the nose are increased.

This may well be the case in congenital underdevelopment of the nose, after injuries were the support cartilage has been damaged, after surgery which has led to loss of support, or when too much tissue has been removed.

n general terms, the proportions of the nose compared to the rest of the facial features are subject to significant personal interpretation, although a number of measurements have been designed to define balance and beauty. The primary elements to be assessed are the height of the bridge, the projection of the tip and whether there is an element of lengthening of the nose (correction of a tip that has become too upturned).

Depending on whether there is enough cartilage still inside the nose – in the nasal septum -to operate the changes, or whether there is no more source for cartilage graft within the nose, it may be necessary to resort to other sources for building up the nose. The options include using bone, cartilage from the rib, using your cartilage, or implantable materials which can be either artificial or biological.

Ethnic Rhinoplasty

In essence, all rhinoplasties are ethnic rhinoplasties, because they take in consideration ethnic characteristics of the nasal shape, skin and facial proportions.

The term has been associated however more with rhinoplasty for population groups with softer nasal contours, like the Afro-Caribbean, the Far Eastern or Pacific Rim populations. Specific features for this nasal shape include often a lower and wider bridge of the nose, wider nasal tip and nostrils (alar base), relatively soft support to the tip of the nose and thicker nasal tip and nostrils skin. Characteristically, for patients that want just a degree of narrowing of the nostrils, an alar base reduction can achieve a satisfactory correction.

Post-traumatic Rhinoplasty

Injuries to the nose, frequently lead to damage to the bone and cartilage support, and change the appearance of the nose, resulting in deviations, asymmetries and sometimes airway problems.

Deviations of the bones and cartilages of the nose, can be reset with relatively good success rate within the first week to 10 days after the injury.

The main worry is that injury to the nose can generate bleeding on both sides of the nasal septum, and the blood clots that accumulate around the cartilage deprive it from blood supply and the cartilage then dies. This situation referred to as septal hematoma, can be treated as an emergency, and when the haematoma is evacuated promptly after injury, the cartilage in the septum is protected.

Loss of the cartilage support from the septum under the bridge of the nose, creates the saddle nose deformity or boxer nose deformity.

A Transformer Star Transformed

Since her breakout role in Transformers, it seems as though Megan Fox has under gone quite a bit of a transformation herself. Once Megan Fox stepped into the limelight of Hollywood her stunning good looks branded her a sex symbol desired by most men. She wore the crown of “the most beautiful woman in the world” for quite some time- and with a title like that why would you ever want to change anything about yourself?

However, through the years we can’t help but observe her looks have definitely been changing leaving many fans upset and asking, “What did she do to herself?” It is more than just a vamped up image that she accredits to womanhood, but a severely enhanced facial structure.

The Facelift

A facelift is an effective procedure to treat problem areas that are present in the lower facial region. Facelifts remove the excess fat, tighten underlying muscles of your face and re-drape the skin around the neck and lower half of the face.

This procedure is typically performed in conjunction with other plastic surgery procedures such as chemical and laser resurfacing, Botox and upper/lower blepharoplasty which typically focuses on the eye area. It is imperative to comprehend that although facelifts offer a profound rejuvenation for your face, they do not correct the texture of your skin that has been damaged. A facelift’s true focus is on the sagging of the skin and how to lift it back to it’s original, youthful position.

A traditional facelift would require an incision that begins in the hairline near the temple and continues in the front of the ear, around the ear lobe, behind the ear and back into the hairline. The Mid-Facelift

The mid-facelift is an effective, minimally invasive procedure that focuses on the areas in between the forehead, eyelids, neck and jaw- the three lift-able areas of your face. Which Injectable Filler is Right for You in NYC?

There’s literally a fine line between having an older fatigued appearance and a youthful rejuvenated appearance. Fine lines and wrinkles are all part of the aging process but that doesn’t mean we have to surrender to them willingly. Sometimes all we need are a few subtle enhancements to revive or retain a youthful, refreshed appearance. Non-surgical, minimally invasive procedures are the way to go when fighting the war against wrinkles and fine lines. It is important to be educated about which filler is best for you and the area in which you wish to target. With a variety of injectable fillers to choose from Dr. Kaplan uses his professional expertise to reverse the signs of aging all while obtaining a smooth and natural look. Here is the breakdown:

Belotero Balance is a hyaluronic acid dermal filler designed to treat superficial lines and moderate to severe wrinkles on the face. It is very natural looking and adds subtle volume and plumpness to facial tissue with little to no downtime. Since the gel is softer than most other products on the market, it integrates into facial skin more easily and adapts well to facial contours. Patients treated with Belotero® have stated that they see immediate results with reduction and softening of wrinkles but the results are more natural and subtle when compared with other hyaluronic acid fillers.

Uses:  BELOTERO BALANCE yields a soft and cohesive gel that, once injected, instantly fills the skin to help smooth moderate-to-severe etched-in lines and wrinkles.4,5 While the product is versatile enough to handle moderate nasolabial folds (smile lines around the mouth), it may be an ideal option for treating delicate areas such as etched-in smile lines and vertical lip lines (thin lines that appear above and around the lips).

Restylane can be used to add volume and fullness to the skin to correct moderate to severe facial wrinkles and folds, such as the lines from your nose to the corners of your mouth.

Uses: Restylane is used to improve moderate to severe facial folds and wrinkles. It can also be used to enhance the lips.

Juvéderm is the first smooth consistency gel formulation and only hyaluronic acid dermal filler approved by the FDA to last up to one year from initial treatment. Injectable gel is injected into areas of the facial tissues to correct moderate to severe facial wrinkles and folds, temporarily adding volume to the skin. Most common areas that Juvéderm provides smooth long lasting corrections are in hollow places on one’s face such as around the nose and mouth as well as scars and is used as fillers for lip augmentations.

Uses: Juvederm Ultra is good for fine lines and not so deep wrinkles of the face. It’s also good for adding volume to facial tissue. More so than its heavier/thicker counterpart, Juverderm Ultra Plus, Juvederm Ultra can be used closer to the surface of the skin. Additionally, since Juvederm Ultra is the lighter of the Juvederm versions, it is good for areas with thinner skin, such as the area under the eye.

Radiesse is an injectable filler that not only provides immediate volume to your skin to diminish the signs of aging, but it stimulates the production of your own collagen to grow. As time passes, the gel is absorbed and the body metabolizes the CaHA microspheres leaving behind only your own natural collagen. The results are proven to last up to a year.

Uses: Radiesse is used for the treatment of moderate to severe facial wrinkles and folds, such as the nasolabial folds. It can also be used to plump up cheek bones. It is not used in the lips.
